Lost or Damaged Passport The following documents must be forwarded (original form) to the Department of Home Affairs, Pretoria, via the Embassy in Muscat: • BI-73 - Application for a passport or travel document • Police report • BI–9 - Fingerprint form • BI-529 - Determination of citizenship status • Copy of current passport (if available) • Copy of Identity document • 2 x passport size photographs (click here for details) Notes on BI-73 and BI-9: Both the application for passport and fingerprint form must be completed at the Embassy. Notes on BI-529: Personal particulars of applicant must be entered in sections A & B, and the parents of the applicant must complete sections C & D. Fees: Are subject to change and applicants should check with the Embassy when they apply. It takes approximately three (3) to four (4) months to issue a new passport.
